1.1 Life threatening

rapidly growing (galloping) malignant disease:

rapid deterioration of symptoms or performance status in time; severe symptoms of progressive disease (e.g. bleeding disorders, sudden dyspnea, altered mental status)

on site, remotely
acute oncology emergencies: central nervous system metastasis, transverse spinal cord lesion, ureteral obstruction, tumor lysis syndrome, hypercalcemia, acute coronary syndrome, ileus, upper vena cava syndrome, heart tamponade, pathologic fracture on site, base medical knowledge could be taught remotely
acute and subacute adverse effects of cancer treatment

(allergic reaction, febrile neutropenia, autoimmunities)

on site, remotely

1.2 Delayed complications

progressive malignant disease

subacute and chronic adverse effect of cancer treatment

suspected (unconfirmed) malignant neoplasm

remotely

+ self-study

1.3 The most common (ordinary)

diagnosed malignant neoplasms, esp. solid tumors: breast cancer, prostate cancer, colorectal cancer, lung cancer, endometrial cancer, gastrointestinal cancer, renal cancer, other gynecological tumors, sarcomas, central nervous system cancer, cancer of head and neck, thyroid cancer, hepatocellular carcinoma, neuroendocrine tumors, germ-cell tumors, cancers of unknown primary origin etc. remotely

+ self-study

systemic symptoms of malignant disease

(e.g. lack of appetite, weight loss, defecation pattern change, hemorrhage, lump, pain);

specific symptoms of malignant disease (e.g. breast lump, dyspnea, hemoptysis);

paraneoplastic symptoms (hematological, neurological, endocrine, cardiological)

remotely

+ self-study

accompanying symptoms of malignant disease (e.g. cachexia, coagulation disorders, depression, eating disorders, defecation disorders, dyspnea, fatigue, nausea) remotely

+ self-study

adverse effects and complications of cancer treatment (e.g. infection, alopecia, bleeding disorders, cardiotoxicity, central venous catheter management, fatigue, hepatotoxicity, endocrinopathy, infertility, impotence, lymphedema, neurotoxicity, CINV, mucositis, pneumotoxicity, secondary malignancies, skin toxicity) remotely

+ self-study

2. Skills (practical and non-technical)

2.1 Basic

The graduate takes a history from a patient (past medical history, drug history, family history, social history, etc.) with regard to malignant diseases and genetic predisposition. on site
The graduate performs general physical examination. on site
To confirm a malignant disease, the graduate suggests a suitable means of diagnostic testing (histopathological verification). He/she understands how to interpret histopathology results. on site, remotely
The graduate suggests adequate laboratory testing and diagnostic imaging. on site, remotely
The graduate determines diagnosis of malignant disease by analyzing acquired data and information and proposes an appropriate differential diagnosis (diagnostic skills). on site, simulation, remotely
The graduate determines type and stage of tumor (using TNM classification) and he/she is acquainted with International Classification of Diseases and Related Health Problems (ICD). remotely
The graduate is acquainted with standard treatment procedures, treatment modalities and their sequencing (neoadjuvant, adjuvant, palliative). remotely
The graduate communicates with the patient in an appropriate manner, he/she treats the patient with dignity and respect; he/she communicates with the patient truthfully, sensitively and clearly about his/her health condition and possible treatment options, with respect to their personality and education. Also the probable next steps and the possible progress of the disease are conveyed to the patient sensitively.   on site, remotely
The graduate knows how to convey bad news (infaust prognosis, disease progression, curative vs palliative intent, termination of cancer therapy). on site, remotely
The graduate is familiar with the individual parts of the medical records and understands their meaning. on site
The graduate is able to search for relevant information on clinical practice of diagnosis, treatment and prevention on both national and international level (guidelines). remotely
The graduate is familiar with the principles of primary and secondary prevention of malignant diseases, with the available preventive screening programs, its requirements and indications. remotely
The graduate is familiar with the basics of follow-up care. remotely
The graduate understands the management of best supportive care. remotely

2.2 Routine

The graduate proposes a suitable treatment algorithm (decision making skills) according to the treatment intent (curative vs palliative), patient’s wishes and his/her performance status; he/she assesses patient’s prognosis. on site, remotely
The graduate is familiar with treatment response assessment (RECIST, iRECIST). remotely
The graduate is acquainted with the roles of particular health professionals involved in the process of diagnosis and cancer treatment. He/she is familiar with an oncology team (tumor board) and he/she knows the roles of all members the multidisciplinary health care team.   on site, remotely
The graduate proposes suitable options of best supportive care in oncology. remotely
The graduate is acquainted with basic principles of palliative care, he/she is able to furnish the patient with advice about the institutions and organizations providing palliative care (e.g. hospices). remotely
The graduate is willing to communicate about death and dying, he/she understands the concept of “good death”. remotely, on site
The graduate is able to search for relevant information, assesse the relevance of the information in question and to distinguish between commercial information and data acquired from impartial sources. remotely

2.3 Optional

The graduate is familiar with the basic types of unconventional therapeutic procedures, he/she understands the reasons that lead patients to prefer alternative therapies, he/she is able to formulate their opinion on these methods pragmatically but without giving rise to a conflict which could induce the premature termination of a cancer treatment. on site, remotely
The graduate proposes suitable palliative intervention while taking the type and stage of tumor, physical and mental condition as well as personality of the patient into consideration. remotely
The graduate is acquainted with possible reasons of cancer treatment termination, he/she is able to consider appropriateness of the termination of a treatment in specific cases. remotely
